Please review the following information & submit the following release PRIOR to your dancer attending any in person class at Muncie Ballet Studio. Dancers WILL NOT be allowed into the building without a submitted release form.

 

DROP OFF PICK UP

  • Covid-19 Waiver must be submitted before attending in person classes.

  • Parents remain in car unless dancer is under the age of 6

  • Lobbies are closed to everyone except staff and students when entering/exiting. No loitering/waiting in lobbies.

  • Arriving on time for drop off and pick up is ESSENTIAL. Early arrivers should wait in the car until it is time to enter (five minutes prior to class time). Parents should be early to pick up dancers so that they are not waiting after class.

  • Entrance is lined with marks indicating where dancers and/or their parent waits until the dot in front of them is vacated while children are entering the building to help maintain social distancing.

  • No changing at the studio, dancers must arrived dressed for class.

  • Everyone must wear a mask while entering & exiting the building.

  • Dancers are required to wear masks while in class. 

  • Dancers and staff must use hand sanitizer upon entering and exiting the building, before and after shoe changes, and before and after any bathroom breaks.

 

HEALTH SCREENING

  • Parents are required to conduct own health screening on their dancer EACH TIME they come to the studio.

    • 1. Does anyone in my household have a cough, difficulty breathing, or a fever over 99?

    • 2. Does anyone in my household have any symptoms associated with Covid-19?

    • 3. Is anyone in my household ill?

    • 4. Has anyone in my household traveled overseas or to a high-risk state over the past 14 days?

    • 5. Has anyone in my household been in contact with a person with Covid-19 in the last 14 days?

    • Answering YES to any of these questions means my dancer should not attend in person classes and should stay at home/take class via Zoom.

  • Staff, students, and parents MUST REMAIN HOME if anyone in their household has a cough, difficulty breathing, or fever greater than 99.

  • IF ANYONE IN YOUR HOUSEHOLD IS FEELING ILL, STAY HOME.

  • Anyone who has traveled overseas or to a high-risk state in the last 14 days may not enter the building and must quarantine at home.

  • Anyone who has been in contact with a person with a case of COVID-19 in the last 14 days may not enter and must quarantine at home.

HOW WE ARE PROTECTING YOUR DANCER AND OUR STAFF

  • Floors mopped and disinfected daily.

  • All surfaces sanitized daily.

  • High contact surfaces sanitized after each class.

  • All once a week classes are 45 minutes. This extra time between class will be for dancers to enter, exit, or switch classes and for instructors to clean high contact areas in their room.

  • Daily staff temperature checks prior to starting work.

  • Dancers must bring their own labeled water bottle.

  • Hand sanitizer provided when entering and exiting the building and each class.

  • Hand sanitizer provided before and after bathroom use.

  • Touchless water faucets have been installed in the bathroom.
     

DURING CLASS

  • Instructors/staff and dancers will be wearing masks.

  • Your dancer will be provided with their own 10 by 9 dance space to maintain social distancing

  • No hand held props

  • Sanitize hands before and after shoe changes

  • No use of mats, lifts, partnering, etc.

  • No food allowed, only water in a bottle brought from home

  • Dancers must only bring what they need for that class or classes and carry it with them.
     

REMINDERS

  • Maintain social distancing at all times.

  • Avoid all physical contact.

  • ANY ITEMS LEFT at the studio will be thrown away. We will NOT maintain a lost and found.

  • Credit card payments may be made online in the tuition portal use MBS1801 as your passcode. Cash and check payments can be made in the tuition box mounted on the wall under the video monitor. Please put cash in an envelope that is clearly labeled with your student’s name. The mailbox is monitored with security camera.

COVID-19 Release

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person to person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited large groups of people.

Muncie Ballet Studio, MBS, has put in place preventative measures to reduce the spread of COVID-19; however, MBS cannot guarantee that you or your child will not become infected with COVID-19. Further, taking classes from MBS could increase your risk of contracting COVD-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child or I may be exposed to or infected by COVID-19 at MBS and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming to or infected by COVID-19 at MBS may result from the actions, omissions, or negligence of myself and others, including, but not limited to, MBS employees, volunteers, and any other customers of MBS. 

I warrant, represent and swear that to the best of my knowledge: My child or I have not in the last fourteen (14) days had any fever, cough, fatigue or other symptoms commonly associated with COVID-19 prior to entering the studio.

My child or I have not been in contact with any family member or social acquaintance who has had any fever, cough, fatigue, or other symptoms commonly associated with COVID-19 prior to entering the studio. 

I warrant, represent and swear that should myself , any family member or My Child come in contact with a person infected with Covid-19  online classes will be utilized for 14 days prior to entering the studio.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child or myself (including, but not limited to, personal injury, disability or death), illness, damage, loss, claim, liability, or expense, of any kind, that I or anyone accompanying me may experience or incur in connection with my presence or any staff members (teachers) at MBS .

Claims”). On my behalf, and on behalf of all those attending with me, I hereby release, covenant not to sue, discharge, and hold harmless to MBS, its employees, agents, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating there to.

By signing this e-form I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of MBS, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after visiting MBS.
